If you are looking for the Connecticut Track Kit site for Survivors then you are in the correct place.

Do you have the correct Username and Password?

Your Username and Password are located on your Kit Tracking Instructions Sheet from your sexual assault kit. It should have been given to you when you completed your kit.

Your Username is the barcode from your kit. It is in the format of CT-123456 and needs to be entered in full to access the site.

Your password is randomly generated and needs to be entered exactly as seen on the instruction sheet. When you enter the site for the first time it will allow you to change your password to something you can remember.

I can't find my Kit Tracking Sheet with my information?

You may request your medical records from the facility that collected the kit, they should have a copy in your file. You may also reach out to DSS.SAKTracking@CT.gov in order to find your barcode and password.

Forgot your password?

If you have already logged in and created your own password you may request a password reset by entering your username (email address) for your account.

Note: Password reset expires after 72 hours.
